Baby death case review: ‘opportunities were missed’
Lee Davison, 28, was jailed for eight years in July for causing or allowing the death of his daughter, Krystal, at Ashleigh Avenue, Pontefract.
A serious case review by the Wakefield and District Safeguarding Children Board (WDSCB) found Krystal’s death last April could only have been prevented if she had been removed from the care of her parents.

Hide AdBut the report said professionals involved in assisting the family were not aware of Davison’s violent history.
It stated: “Opportunities were missed in regard to identifying the level of risk to Krystal from her father’s history of domestic violence and the evidence of physical abuse of previous partner’s children.
“If all the relevant and significant information had been collated and understood more adequately, it is highly probable that Krystal would have been subjected to formal child protection (and) contact with her father would have been managed differently.”
